Hotjar On-site Surveys can be used to recruit user testers from visitors to your site.
Visit your Surveys page and click on New survey.
Enter a name for your Survey.
Select the Location of your survey as either Popover or Full Screen as the location.

A small widget shown on your webpage. Users can continue to interact with other content on the same page. - Full Screen
Displays as a larger overlay window in the center of the page. The background content is greyed out so that your survey is the main focus.
As the first question type, select Statement from the dropdown menu.
The Statement question type allows you to invite website visitors to participate in testing. It does this by displaying custom text without providing a response field or answer choices. Statements contain a title field and an optional description field. For example, you could offer a prize to get a user's attention. For example, "Want to win a $20 gift card by helping us improve our site?"
Add further questions to better understand your testers and request their contact information.
Short text answer and Radio button question types are great for asking your users specific questions to see if they're the tester you're looking for.

Finish configuring your Survey as desired.

Click Create Survey.
To ensure the Survey starts collecting responses from user testers right away, make sure to change the status to Active before clicking Create Survey.
